We ask you to inform us in a timely manner of any changes to insurance details and/or personal data.

• You are requested to bring your own towel. Furthermore, we kindly ask you to observe personal hygiene rules.

• FysioDiamondFactory supervises physiotherapy interns. If you do not wish to be treated by an intern, you can inform your physiotherapist.

• The practice is not responsible for theft of belongings in the building or outdoor area.

• At the end of the treatment process, you will be invited via email to complete a patient satisfaction survey. This directly helps in the continuous improvement of our physiotherapeutic care. If you have any objections, you can inform your physiotherapist.

• Registered data will be processed in accordance with the General Data Protection Regulation (GDPR).

• If you are not satisfied or have a complaint about the treatment by your physiotherapist or another staff member, you can report this to your physiotherapist or the practice owners Menno de Vries or Regi Severins. Your complaint will be handled in accordance with the Healthcare Quality, Complaints and Disputes Act (Wkkgz).

• You are responsible for communicating a do-not-resuscitate declaration.

• Smoking is prohibited in all areas.

• Pets are not allowed.

• If you have comments, ideas, etc. for improving our services, we would greatly appreciate it if you could let us know. You can communicate this to us both verbally and in writing.

• You must be aware of and agree to the house rules of physiotherapy.
